Completion of Medical Forms
Medical Forms from 3rd parties may take 4-6 weeks to complete as per the OMA guidelines. All forms must have the patients information and signature completed before handing in to the doctor.

Songbird Medical is committed to providing a safe and respectful environment for staff and patients. There is a zero-tolerance policy for threatening or inappropriate behaviours towards doctors, staff, or any other patients. This includes, but is not limited to, swearing, yelling, threats or threatening behaviour, rude or demeaning comments, sexual harassment, and discriminatory remarks. Any patients engaging in such behaviours in person, by telephone or video, will be immediately discharged from the practice.
